This product is restricted to industry employees only, with a valid occupational license. It includes:

• Occupational License credential printed on a white PVC card
• Vinyl transparent credential holder at no additional cost

Simply upload your digital license by pressing 'Attach Document'. Read on for tips to help you access your document:

Search your email inbox filtered by the dates you were processing your license. If you can't find the file, access the Dept. of Health platform and refer to the case number assigned when you submitted all your documentation for licensing or renewal. 

You will receive your card by mail, via USPS in approximately  5-7 business days.
